All PTA Sponsored Activities and Clubs MUST submit an Income Claim Form for any/all money received.
You must also submit an Expense Reimbursement Request to receive compensation for expenses incurred.

The following forms are required by Fairfax County before we can be allocated any space at the school.
Please fill them out and return to the school office. If not filed, we will lose our space at the school.
Each activity must fill out a form.

(Required by ALL clubs and activities)
Club /Activity Final Financial Statement (ADM-24A)
Must be filed within 4 weeks of the last session.
(Required when paying FCPS employees)
